How can you have a mayor for a region?

A BOROUGH Mayor?

A mayor of Tavistock. A mayor of Okehampton. Of Torrington, yes! But a mayor of a region? How could an individual persuade inhabitants of separate communities that he would rule over several with impartiality?

Each town has its mayor. Why not elect that one rather than create another over a group of different communities?
